THE MEDICAL MILK OF HUMAN KINDNESS

September 26, 2016. I was standing this morning at General Bert’s bedside. He was pretty close to unconscious and may have septicemia again (infection in the blood) although different blood cultures disagree with one another. His Infectious Disease doctor and I were discussing his status when another doctor, this one in a pink Izod-type shirt, came in and started talking.
.
An amazing thing happened next (before the next amazing thing): he apologized for interrupting our conversation and actually waited until we turned to him and included him in our conversation.
.
He is the ENT (Ear, Nose and Throat) doc who wanted me to sign a consent form for a tracheostomy (trach) surgery on Wednesday.
.
If you have been reading this blog for a while you know that I have fought like a tiger to avoid a trach for Gen. Bert.
.
Now, however, the situation is different: he is breathing pretty well on his own (YEA!!!) but he is so weak that he will drown in his own secretions if there is no easy and accessible way to perform what is known as “pulmonary toilette”, or tapid and easy suctioning of his airway until he is strong enough to swallow and cough on his own.
.
So I signed (adding “All Vaccines Refused” to the consent form, of course).
.
Amazing thing #2: as he was leaving the room, the doctor stopped, turned back to me and said, “You are doing the right thing. He will be fine, you know.” and left.
.
I followed him out of the room and tapped him on the sleeve and said, “Dr., I want you to know that the moment you took to offer me comfort and support was very meaningful to me. Thank you.”
.
He, and apparently the 4 or 5 physicians standing around him, appeared taken by surprise.
.
Now, there have been other doctors who have been both human and humane here. I have mentioned Dr. Richard Menashe before as a shining star of competence and kindness.
.
Help from many quarters. Last night, during a discussion with the daughter and son in law of a lady who has been here on the CCU since shortly after New Years, the daughter, who is a dialysis nurse with decades of experience, mentioned a nutritional approach called IDPN (Intra Dialysis Peritoneal Nutrition).
.
I was not aware of IPDN by Pentec which is delivered during dialysis and raises the albumin level of patients who receive it.
.
You see, many of General Bert’s problems come from the fact that an important protein in the blood, albumin, is catastrophically low.
.
That means that fluid is literally leaking through his skin, from his blood vessels into his tissues and into his chest cavity.
.
He needs protein/albumin BUT the kidneys cannot handle much of it. Thus, we nephrology tango.
.
But with IDPN, apparently protein is given that the body can hold on to.
.
I got very excited learning about this and called General Bert’s nephrologist early this AM and now, through his cooperation, this strategy will begin for General Bert on Wednesday.
.
I have great hopes that more protein will give him more balance which will give him the ability to hold his fluid inside his blood vessels and stop leaking it.
.
That will go a LONG way to helping him rebound.
.
I am, of course, grateful for the chance conversation that taught me about IDPN!
.
Once again, the kind community of shared experiences offered options that can be life saving.
.
If it turns out to, in fact, BE lifesaving, we will be in a very different place in just a few days.
.
We shall see.
.
